Telehealth - How to clock in

Below is a recap of general instructions for clocking in under Com-Hab.


Further down, are additional requirements for Telehealth sessions to ensure all steps are completed correctly.



  1. Once logged in to the app, select the “Service Delivery” app.

  1. Select "New Session” on the bottom right side of the page OR the "+" on the upper right-hand side of the page.

  1. SelectSDS-Self-Hired Community Habilitation”.

    1. If this is not available, please reach out to FIHR@Yedei.org.

  1. Select “Mode of Support” and then select "Telehealth".

a. Note: Telehealth services can only be selected if prior approval was received from the FI.

  1. Select the "Individual" that will be receiving services.

    1. If the individual you are working with is not available, please reach out to FIHR@Yedei.org.

  1. Select "Done" in the upper right-hand corner.

  1. A pop-up will appear with the message "Pending Monthly Summary" - Click "Yes" to proceed.

  1. A pop-up will appear to select the "Department" & "Reason".

    1. Select "Self-Hired Comm Hab".

    2. Select “Direct Service”.

      1. Note: the “Overnight” option cannot be chosen for Telehealth services.

    3. Select “Done” on the top right-hand side.

  1. Select the “Start” button to start the clock.



Staff are only clocked in once steps 1 - 8 are completed.



Below are additional instructions for clocking in under Telehealth.



  1. Once fully clocked in, select the “Telehealth Session” button at the bottom of the screen.




  1. A pop-up will appear. This section can be completed now or at the end of the Telehealth Session. If completing this section now, select the "Telehealth Type" and fill out each field below.


  1. Describe the call – Note: The following description can be used: “See Service Documentation for information.”


  1. Select "Save and Close" to proceed.



Note: For Phone Calls, do not select "Dial Now" as the system may not function correctly.


You are now fully clocked in.


When clocking out, please refer to "Telehealth - How to clock out".
