Reimbursements are allowed for up to 24 months, which do not need to be consecutive. No reimbursements will be issued after the 24-month limit. Cannot exceed $800 per month or $250 per class.

“Attachments”

• The Legal Name of the individual must be included. If the individual has a preferred name, and it is listed in their current Life Plan, it can be used as well.

• If the same attachment is going to be submitted for multiple months, the number of invoices should be listed.

  • For example – A program with attendance from Jan – March; if 3 eVero invoices are submitted using the same attachment, starting with the first eVero invoice, the description should say “1/3 invoices” and subsequent invoices should say “2/3 invoices”, and “3/3 invoices” respectively.
