Camp

Prior approval is required for each season and/or calendar year, even if the exact same camp is attended.

An active NYS Health Permit/Certificate is required annually, even if approval was received for the same camp in previous years. This applies to winter and summer camps, etc.

The attachment in eVero needs to include the individual’s legal name. If the individual has a preferred name, it can be used, so long as it is listed in the current Life Plan.
