Com-Hab Monthly Summary Instructions

How to enter a Monthly Summary in eVeroMobile

General Information

Monthly Summaries need to be submitted prior to the end of the following month.


  • Monthly Summaries need to be submitted monthly.

  • Only Com Hab staff need to fill out the Com-Hab monthly notes.

  • Only one staff per Self-Directed Person can fill out a Monthly Summary for any given month.

    • If multiple staff work with the Self-Directed person, a rotation of staff can be implemented by the Self-Directed Person/Designee.

  • The following information must be included:

    • How did the staff help implement the valued outcomes?

    • How did the participant respond to the services?

    • Were there any issues or concerns?


Reminder: Monthly Summaries are routinely collected by OPWDD, Medicaid, and other New York State Departments as part of service and policy reviews. Please ensure all information entered is done so in a professional and respectful way.

How to enter Monthly Summaries through the mobile app


Selecting a Monthly Summary:


  1. Log in to eVeroMobile and select the Monthly Summary module.




  1. Select the month and the name of the Self-Directed Person.



The number indicates that 2 monthly summaries are needed for that month. This is because a Staff Action Plan became effective mid-month, which is indicated by the start dates in the image below.

  1. All goals & safeguards listed in the Monthly Summary must be completed.



Services & Monthly Progress


a) Responses

  • The "Responses" button will display all service documentation submitted for that goal, for the selected month. Reviewing the responses can help with summarizing how each goal was worked on by staff. Otherwise, continue to #4.



b) Monthly Progress

  1. Select the "Monthly Progress" button next to each line.




  1. "Monthly Progress" - Select one of the options from the drop-down menu.

  2. "No Update Required" - This option can only be selected if the Valued Outcome/Goal was not worked on that month or if the line is a Safeguard Support/Goal.



  • "Service" - This field shows the goal.

  • "Individual Response" and "Staff Response" - These fields are automatically generated from the daily Service Documentation entered every session that month.


  1. "Service Summary" - Must have information entered to move forward. Select anywhere in the text box to add information, based on the following:







  1. Select “Done” in the upper right-hand corner to return to the list of Valued Outcomes/Goals.




  1. Continue to complete the remaining Service Summaries for all the Valued Outcomes/Goals listed.

    1. Note: Once a Monthly Progress is completed for a Valued Outcome/Goal, a letter will be displayed in the yellow circle. Definitions for each letter can be found at the top.




Issues & Concerns


  1. Answer the "Issues & Concerns" questions. Select from the drop-down options for each question.

    1. If "Dissatisfied" (with supports and services), "Has" (had significant life changes or events), or "Are" (health and safety issues) is selected, a comment is required.

    2. Additional Comments - Does not need to be filled out but can be used if more space is needed for any of the "Issues & Concerns" questions.




Monthly Summary Note


  1. "Monthly Summary Note" - Select anywhere in the white square to add information.

    1. The Summary section should be filled out with an overall summary of how the staff implemented the Staff Action Plan during that month.

    2. When finished, select “Save” to add the information.




Response to Service


  1. Staff can see the Service Documentation they entered for each session they worked on during that month, by selecting the “View Comments” button.

    1. Note: this section only populates once the 3 questions in step 10 have been answered.

    2. This can be viewed prior to entering the Monthly Summaries for each Valued Outcome/Goal to ensure that the correct information is added.



Signing the Monthly Summary


  1. Select the “Sign” button when the Monthly Summary is ready to be reviewed by the FI.

    1. The Monthly Summary should only be signed once the month is over.




Monthly Summary Statuses


  1. "Doc Status" - will show the status as either “Draft”, “Signed” or “Approved”.

    1. "Signed By" - will list which staff signed the summary.

    2. "Approved By" - will list the FI coordinator that reviewed and approved the Monthly Summary.

    3. "Rejected By" – This section will only appear if the Monthly Summary was rejected and will list the FI coordinator that reviewed and rejected the Monthly summary. The reason for rejection will also be listed.



  1. Once the summary is signed or approved, the color of the document on the main Monthly

    Summary screen will change. There is a key on the page that explains what the different colors mean.
