How to Enter Service Documentation Post Session

If staff did not use the eVero mobile app to clock-in, uses Telephony FOB, or entered the hours incorrectly, Service Documentation must be entered in eVero after the session was completed/corrected.


There are two ways to update the service documentation:


  1. Mobile App

  2. Website


See below.



Mobile App


  1. After logging into the mobile app, select the “Service Delivery” icon.


(Ensure that the “Community Services” tab is highlighted.)


  1. Select the “Missing Documentation” tab if no services were documented during the session.

  2. Otherwise, select the "Today", "This Week", "Last 2 Weeks" or "This Month" tabs to locate the session that needs editing.


Note: service dates outside these date ranges can only be edited online. See further down below.



  1. Select the session to begin editing.

    1. Note: If there is a green “Thumbs Up” symbol, it means that the session has been reviewed by the Individual/Designee and cannot be edited. Reach out to FI@Yedei.org to have the session un-reviewed so it can be edited.



  1. Once the session information populates in the Service Summary, select the “Services” icon at the bottom.



  1. Follow the steps 2 – 7 in “How to enter Service Documentation during a session”.


  1. Once all edits have been made, a pop up will confirm the data has been saved successfully.




Website


Begin by visiting myeveroportal.com​ and logging in with the same username and password used to log into the eVero mobile app.


Note: editing services can be done from a desktop or a mobile device. It is recommended to use a desktop since the mobile website may not be configured properly for your device and may cause issues.


  1. Once you've logged in to the eVero website, select the “SDS” dropdown menu and then select “Attendance Roster.”


  1. "Date Range" – Enter dates for a session that is either missing Service Documentation or needs to be edited.

  2. Program” – Select “SDS-Self-Hired Community Habilitation”.



Note: At this point the page will load all sessions for the date range and program selected.


  1. Individual” – Does not need to be selected however, it can be used to narrow the search, if desired.

    1. If selected, press the “Refresh” button to reload the table.

  2. To edit the service documentation, select the number in the “Service Count” column.



  1. A pop-up will appear with a list of Valued Outcomes to choose from.

    1. Only “Habilitative Goals” or “Habilitative Support” lines can be used for billable services.

  2. SelectView” to enter/edit the services documented on that specific date. If there is no “Edit” button, reach out to FI@Yedei.org.



  1. Complete each required section. If necessary, refer back to steps 3 – 7 in “How to enter Service Documentation during a session” to complete this section.

  2. Once all the information is filled out, select the “Save” button at the bottom of the page.

  3. Select another goal to edit or press "Close" to close the window.


The session can now be reviewed by the Individual/Designee.
