Medicaid Service Coordination

Yedei Chesed’s Medicaid Service Coordination program provides individuals with developmental disabilities the opportunity for an improved quality of life and the potential for independence. Inclusion is our goal when it comes to individuals with disabilities. Our Medicaid Service coordinators advocate for the rights, choices and opportunities of our clients. We do our utmost to enhance their production, individuality and to enable them to reach their fullest potential.

Service Access Assistance

Service Access Assistance allows families with a child who has a developmental disability and do not qualify for Medicaid, to receive the myriad benefits of service coordination. This program allows all families, regardless of financial situation, to access the resources of Yedei Chesed.

Plan of Care Support Services

Plan of Care Support Services provides families with little current need with the opportunity to keep their file active even though they do not need the full time case management opportunities of traditional Medicaid service coordination. By choosing this option, families are able to keep their options open for future services.
